Access Health Care Long Island Coalition

The Health & Welfare Council of Long Island has secured funds through the New York State Health Foundation to administer a bi-county workgroup, the Access Health Care Long Island Coalition, comprised of both the Nassau and Suffolk County Departments of Social Services, Facilitated Enrollment lead agencies, managed care organizations and community-based organizations.

 

Access Health Care Long Island Coalition (AHCLIC), a non-competitive, collaborative workgroup of high level staff involved in all aspects of facilitated enrollment, will create logical systems for enrolling, retaining, and efficiently serving managed care enrollees while meeting all regulatory requirements. 

 

The workgroup's objectives are:

  • to increase the number of Long Island individuals enrolled and retained in one of the State's programs,
  • to decrease the error rate of eligible individuals found ineligible for "non-documented" compliance, and
  • to decrease the time it takes for determinations to be made and communicated.

RECENT NEWS
AHCLI Coalition members created a Change of Address Form that can be used by individuals with active Medicaid, Child Health Plus or Family Health Plus cases when they have moved.  The form should be completed and mailed to their local Department of Social Services with in 30 days of the move.  Click here to download the Change of Address Form.
