June 16 – HWCLI Annual Membership Meeting

Please join us for HWCLI’s Annual Membership Meeting!

Thursday, June 16, 2016 | 8:30am
Crest Hollow Country Club

HWCLI is pleased to announce this year’s awardees:

Ned Wight
Unitarian Universalist Congregation at Shelter Rock
The David Kadane Memorial Award for Advocacy
Ned Wight, who has served as Executive Director of the Unitarian Universalist Veatch Program at Shelter Rock for nearly a decade, will retire in July 2016.

The Unmet Needs Roundtable Donors:
Dale Cole, Marian Conway, Warren Ferry, Jana North
The Sandy Lenz and Jim Harnett Community Service Award
Since the early Spring of 2013, these four donors have served weekly at the Unmet Needs Roundtable to hear cases presented by Disaster Case Managers on behalf of those impacted by Superstorn Sandy. Collectively, they have provided millions of dollars to address Long Island’s unmet needs.

Agenda
We continue our conversation on DSRIP and the role of CBOs. NYSDOH has released an RFA for a CBO Planning Grant for organizations with budgets under $5 million and not receiving Medicaid dollars. This is an opportunity for smaller CBOs that play critical roles in addressing social determinants of health, such as housing, child care, and emergency food, to plan for their future participation in DSRIP. As well, it is important for CBOs already engaged in Long Island’s PPSs to share their perspective. This is an opportunity to discuss building a Consortium of agencies to apply for this grant for Long Island.

Nassau and Suffolk Community Based Organizations Large and Small are Strongly ENCOURAGED TO ATTEND AND PARTICIPATE

This breakfast is FREE to HWCLI members (two seats per member organization). Additional seats can be purchased at $35 each.
